Chronic Conditions Clinics

Everyone with a chronic disease, even when well, should attend for regular review of their condition. This is to ensure you continue to remain well and is a vital part of your on-going care. A letter will be sent requesting your attendance. Such clinics are led by our team of highly trained nurses for a variety of conditions such as:

• Asthma
• COPD
• Coronary heart disease
• Diabetes
• Hypertension

Please see the clinic times page in the opening times section of the website for the details. If you have any problems attending the review clinics please tell us - we will try our best to accomodate your needs.

It has been shown that those who do not attend for review of their health conditions are likely to:

• Have more complications of their health problems
• Leading to worsening of their health
• Ultimately more admissions into hospital
• Worse long term health problems

Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to complete a travel questionnaire and hand this in at reception, the nurse will then check if you need any vaccinations. If you require any we will call you to arrange an appointment with the practice nurse. 

We ask that you do this as soon as possible so that we can ensure you get the appointment with the nurse as the nurse clinics do get very busy and we may struggle at short notice.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment maybe required with the practice nurse to actually receive the vaccinations. Some vaccines have to be ordered as they are not a stock vaccine.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Non-NHS Services

Some services provided fall outside the scope of the NHS and therefore attract charges. Examples include the following:

• Insurance claim forms
• Prescriptions for taking medication abroad
• Private sick notes

Our reception staff will be happy to advise you about appointment availability and applicable charges.

If you have a form or letter that needs completing by the doctor don't make an appointment, simply ask at reception first. Often an appointment is not needed.

Passport photographs and applications can be signed by a lot of other people; so, we have stopped doing these

NHS Health Check

What is an NHS Health Check?

The NHS Health Check is your chance to get your free midlife MOT. For adults in England aged 40-74 without a pre-existing condition, it checks your circulatory and vascular health and what your risk of getting a disabling vascular disease is.

If you are aged 40 and over you may be at risk of;

Heart disease

Stroke

Kidney disease

Diabeties

Dementia

The NHS Health Check can help you reduce these risks and make sure that you stay healthy. NHS Health Check is offered and encouraged to patients eligible to enable us to work with you to lower your chances of developing the identified health risks that increases with age.o

What happens at the check?

This check is to assess your risk of developing heart disease, type 2 diabetes, kidney disease and stroke.

The check will take about 20–30 minutes.

You’ll be asked some simple questions. For example, about your family history and choices which may put your health at risk.

We’ll record your height, weight, age, sex and ethnicity.

We’ll check your blood pressure and cholesterol.

What happens after the check?

We will discuss how you can reduce your risk and stay healthy.

You’ll be taken through your results and told what they mean. Some people may be asked to return at a later date for their results.

You’ll be given personalised advice on how to lower your risk and maintain a healthy lifestyle.

Some people with raised blood pressure will have their kidneys checked through a blood test.

Some people may need to have another blood test to check for type 2 diabetes. Your health professional will be able to tell you more.

Treatment or medication may be prescribed to help you maintain your health.r or dementia.
